Payment Information
Payment must be made at the time of the office visit. We accept cash, all major credit cards or private insurance. We do not accept personal checks. Patients are responsible to pay any deductible, coinsurance or any other amount not covered by an insurance plan.

Insurance Accepted
Our practice accepts many insurance plans. Contact our office or your insurance provider to verify that our location is listed as an approved provider prior to your first visit.

Prescription Refills
Prescription Refill requests are accepted during regular business hours, Monday through Friday. Please anticipate your needs and allow a 24-hour turnaround time. Have your pharmacy request refills by fax to 713-861-4093.
