Please call to cancel at least 24 hours prior to a scheduled visit.
Payment Information
Payment must be made at the time of the office visit. We accept cash, personal check, all major credit cards or private insurance. Patients are responsible for paying any deductible, coinsurance or any other amount not covered by an insurance plan.
